All students must maintain at

There is a boy's and a girl's team for each sport.
In basketball only, there is a separate team for sixth grade
boys and girls. Members of the teams are decided through tryouts.
Games are held after school on Mondays and Wednesdays (Track
meets on Wednesdays, only). Team practices will be held daily
at a time decided by the coach (normally after school for about
2 hrs.) Practices for Basketball and Soccer could be held at
St. Luke’s or Dr. Phillips YMCA and you will be required
to provide your own transportation.

Notices
- For soccer, basketball and volleyball, 8 games
will be played – 4 home games at Chain of Lakes Middle
and 4 games hosted by schools within our “cluster” (Chain
of Lakes, Southwest, Carver and Gotha).
- Sixth grade basketball teams will play 4 games.
- The track teams will run in 4 meets, competing
against the teams in our cluster.
- Windy Ridge doesn’t provide bus transportation;
therefore you must provide your own transportation.
- At the completion of each season, there will
be a Festival competition among all the middle schools in Orange
County.

- EMT Card (located in the main office)
- Risk Form (located in the main office)
- Physical Form (located in the main office)
- Athletics

Required Forms
Students must complete all required forms prior
to tryouts.
- A current physical (sports screening) dated
after May 23, 2006. The physical is to be given by a doctor
and all information is to be completed on the form provided
by the school. The new form, (REV. 04/11/02) must be used.
- An Emergency Medical Treatment Card.
- A Notification of Risk Form. (Parent / guardian's
initials required next to each sport child is authorized to
participate in. Also, signature and initials required at end
of form.)
- Addendum to Athletic Sports Screening Physical
Form.
